CLIENT: Use the table above to select the services which you wish to purchase from the Provider before signing this computer service contract. The “total” amount in the table above will be invoiced to you directly once services are rendered.

Terms & Conditions
Warranty
The Provider offers no warranty for the services provided. However, any new or refurbished parts installed as part of this computer service contract may carry their own manufacturers’ warranties. Those warranties are supported by the manufacturers directly. Any claims or inquiries should be directed to the manufacturers offering such warranties.
Payment
The Client agrees to pay the full amount indicated in the table above for the services selected once services have been rendered by the Provider. The Provider shall provide an itemized invoice, which may be paid by cash, credit card, or check.
Customer Data
The Client is solely responsible for securing and backing up any data stored on the computers being serviced. The Provider shall not be held liable for loss or corruption of data for any reason.
Liability
The Client agrees to indemnify and hold the Provider harmless against any and all claims of loss or damage related to the services performed under the terms of this computer service agreement.
